Appointment
The first step towards a beautiful, healthy smile is to schedule an appointment.
Your scheduled appointment time has been reserved specifically for you. We request notice two business days in advance if you need to cancel your appointment. We are aware that unforeseen events sometimes require missing an appointment, and appreciate your cooperation.
In order to ensure quality orthodontic care, it is imperative that our parents and patients understand the manner in which we schedule our appointments. Most parents work and all children attend school. Since the vast majority of our patients are of school age, it is unavoidable that some school is missed even though we make every effort to minimize this. Orthodontic maintenance and adjustment appointments usually take 10-20 minutes; therefore, we reserve after-school hours for these kinds of appointments, since we can accommodate the greatest number of patients in the after-school hours. Longer appointments, repair appointments, and restorative and surgical care appointments are scheduled during school hours and orthodontic patients are not seen on a ‘walk-in’ basis.
We will be glad to work around certain classes that are very important or ones in which your child may be having problems. School excuses are provided for each appointment and it is important for the patient to turn these in. Thus, our afternoon appointments are scheduled so as many patients as possible do not have to miss school. If the patient is a student, these appointments often can be arranged during study period, lunch time, recess, before school, or in-service days.

Emergencies
If you encounter an emergency during our work hours, call our receptionist so that she may schedule you and have the office prepared for your arrival. If you encounter an emergency after hours, call our office and our voicemail
will instruct you on how to page the doctor on call.
Patient Forms
If your child is a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Payment Options
Payment is expected at the time of service unless other arrangements have been made in advance.
We accept Visa, MasterCard, American Express and Discover, and have other financing plans available.
In the event that it is necessary to initiate collection proceedings on your account, you will be responsible for all attorneys’ fees and cost of collection.
THERE IS A $50 (minimum) CHARGE FOR CHECKS RETURNED FOR ANY REASON.

Dental Insurance
Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Your estimated patient portion must be paid at the time of service. As a service to our patients, we will bill insurance companies for services.
If you have private indemnity insurance, we will bill your insurance carrier for you at no charge, provided you have assigned the benefits for the treatment rendered to the office. We will bill up to two insurance earners for any given treatment series at no charge. If there is a need to submit additional forms, there will be a charge of $10 for each additional form billed. If there is an overpayment by your insurance company for any treatment sequence, we will refund the overpayment to you or your insurance company on a regular basis. If it is ascertained that your insurance company has made a payment in error, it will be the responsibility of the insured to provide reimbursement to the insurance carrier.
We expect timely payment for services rendered from your insurance carrier. If there are any delays in payment, we will send you a statement requesting payment in full after 30 days. We will begin to assess finance charges (.833% month / 10% APR) on the unpaid balance 60 days after treatment has been rendered. We encourage you to contact your insurance carrier regarding payment to avoid any additional finance charges.
